I agree with Mark concerning the Learning Needs Analysis form.
In general I thought the questions were relevent to finding out what the learning needs of the students are and the best ways to get the most out of the peer tutoring sessions.
Like Mark, I also don’t think that knowing the highest qualification of each student is relevent to learning needs as it does not necessarily indicate their strengths, weaknesses or ability regarding the subject we are tutoring them in. This information may also come up incidentally in the course of tutoring.
I re-jigged the format of my Learning Needs Analysis before I printed it out so that there is room for answers under each question and I can use one form for each student.
